Jonathan just dies, Joseph loses his buddies, Jotaro loses several friends, Giorno loses his only real friend, Jolyne is erased from existence and replaced with a utopic doppleganger, Johnny loses his only friend, Josuke's mother isn't healed.

Araki’s art declined somewhere in the middle of Jojolion but he is over 60 years old now. It’s still readable even with the scratchy line work. He really peaked in the middle of SBR

Araki definitely does not have the middle and end of a part in mind when he start, but I think it's a little bit extreme to think he has no plan whatsoever. I think that Araki bases each new part somewhat off of the previous part with the intent of building upon them, and I think he does have a grander philosophy regarding humanity, resolve, and fate that permeates the whole series, sometimes intentionally and other times not.
