In an alternative universe where the moderation team was less lazy and/or was willing to talk to anons more often /qa/ could have worked. It took years of neglect for things to get as bad as they did. I mean they could have done a monthly thing where they come and talk to anons and answer a handful of questions, I feel many anons would feel a bit better getting told "no" to any idea/suggestion they had upfront then the anarcho tyranny they experienced cause the moderation team was fickle and there were unwritten rules about things that while perfectly legal on he board would net anons a ban if they so much s hinted at it or clearly broke board rules but was allowed cause a certain mod was protecting it silently. Stuff like this caused extremely bitter resentment, it's miracle the hack didn't happen sooner .

at least here moderation is more local. if a board really does go to shit anons can just pack up and leave to a new one so it incentives BO's to not get too high on their own power. Granted 8chan is still pretty slow so that might be the main reason things aren't unmanageable, I have no idea if this system would work if 8chan had typical levels of 4chan activity daily.
